Letter of consent for a child’s Thai passport request (Thai and non-Thai parent)
The certification of Thai nationals’ signature in a power of attorney and a letter of consent.
***Non-Thais’ signatures can only be certified in the case of a letter of consent for their child’s Thai passport request. For other purposes, please see the certification of non-Thai nationals’ signature
The certification of Thai nationals’ signatures must be done only in the presence of the Embassy officer. Applicants are not to sign the power of attorney/letter of consent in advance.
Required documents for a letter of consent request :
- A letter of consent form
- Documents of the consent giver
- 2 copies of the valid (Thai) I.D. card (certified “True Copy” and signed)
- 2 copies of (Thai) passport (certified “True Copy” and signed)
- (In the case of a Thai national) 2 copies of House registration/ Tabian Baan (certified “True Copy” and signed)
- 2 copies of other relevant documents such as a certificate of name change ( if applicable)
- Documents of the other parent or the authorised person who will take the child to request a passport
- 2 copies of the valid (Thai) I.D. card (certified “True Copy” and signed)
- (In the case of a Thai national) 2 copies of House registration/ Tabian Baan (certified “True Copy” and signed)
- 2 copies of other relevant documents such as a certificate of name change ( if applicable)
- Documents of the child
- 2 copies of the valid Thai I.D. card (certified “True Copy” and signed)
- 2 copies of a birth certificate
- 2 copies of other relevant documents such as a certificate of name change ( if applicable)
- Fee: 15 euros per document (pay in cash ( banknote only) with an exact amount or Bancontact)
- A legalisation application form
